Public Housing Forms

These forms are only for current applicants and tenants at our Public Housing properties or senior buildings (Acacia, Katherine Bates, Prospect Lake, Casa de Cerro, Crestview). None of these forms are applications for the wait list and will not be considered as such. Here you can find Colorado Springs Housing Authority (CSHA) forms to fill out that will be automatically sent to us, and a copy to you. Typically a Housing Authority employee will refer you to here and tell you what form to use.

Enterprise Income Verification (EIV) Brochure (A guide for applicants and tenants on reporting complete and accurate income information)

Public Housing - ACH Authorization for Direct Payment (If you want the CSHA to automatically withdraw your rent from your bank account please print this form, fill it out, and bring into our office or mail it, along with a voided check)

Public Housing - Email Authorization (Use this form to supply the CSHA with your email address)
