Use this packet if your entire household has no source of income whether earned or non-earned.
Authorizes the CSHA to automatically withdraw your rent from your bank account. Please print this form, fill it out, and bring into our office or mail it, along with a voided check.
Provides authorization for the CSHA to email any correspondence needed to determine continued eligibility.
