THIS IS NOT AN APPLICATION. Use this if you are an applicant on our current HCV wait list and you need to update your application or contact information.
A guide for applicants and tenants on reporting complete and accurate income information.
Used to verify eligibility and ensure that benefits are set at the correct level.
Report a HCV program violation. Specific dates and times of the incident are needed for follow-up on the concern. Any violations must have verifiable evidence that can be proven before any adverse action can be taken. The CSHA is not mandated to mediate. complaints
Used to confirm you have read and understand what information the CSHA is required to provide to HUD, who will have access to this information, how this information is used, and your rights.
Must be completed for each family member.
Provides authorization for the CSHA to email any correspondence needed to determine continued eligibility.

Use this to report ALL changes in household income and/or family size. You are required to report all changes within 10 business days from the date of change and/or receipt or loss of income. Spanish Version

Print this packet so both landlord and tenant can fill it out. Use this for requests to move to another unit. All requests must be approved by the CSHA and the unit must pass an inspection prior to the family moving.
Use this packet once you have received a letter from the CSHA Intake Department stating we have received your information from your current Housing Authority. This letter also states your voucher size with the CSHA.
Use this form to request to move your assistance outside of the CSHA area. You must be in good standing with your landlord and the Housing Authority. Your assistance cannot be moved if you owe money to the Colorado Springs Housing Authority.
